Issuing SST Cards
PDH STAR is a New York City Department of Buildings (DOB) approved Site Safety Training (SST) provider and is authorized to issue the mandatory Limited, Temporary, Full/Worker and Supervisor SST cards. We can issue the SST card even if you have completed your training with other provider(s).
To request your SST card please fill out the form (download form here) and either email to email@example.com or fax the completed form to (833) 734-7827 (PDH-STAR). If you have completed any part of your training with other training providers, you need to submit proof of courses taken elsewhere by sending us completed course certificates and / or cards.
The cost of the SST cards is:
Supervisor SST Card $39
Worker / Full SST Card $29
Processing time varies, depending on which provider you have taken your training with, so please allow sufficient time for your training to be verified and the SST card to be issued. Please make sure your application is completed in full with legible block letters – incomplete and / or illegible forms may delay your SST card issuance.
ID and Proof of Address
For us to be able to issue the SST card, the DOB requires us to verify identity via a government issued ID and a proof of address. For eligible types of documents please click here. Please submit a copy of your documents with your application.
PDH STAR needs to verify your training taken through other training providers. For additional details regarding the training, including online training, verification, please click here. Individuals presenting Certificates of Completions or other documentation as proof of their online training are required to sign a document attesting to having completed the training on their own and according to the law. For a copy of the document please click here (to be filled in and submitted at time of SST Card applications).
Please note that, in case your application for an SST card is denied, you will not be able to receive any refund.
